From day one after elective acceptance

Now congratulation now you are accepted in elective program which is very important thing in the residency application, but the most important is to get perfect LOR during elective. So now we will discuss obstacles and steps to get good LOR.
أولا: لابد ان تكون نيتنا نرضى ربنا , علشان السفر و المذاكره و المصاريف اللى هتدفع تكون فى ميزان حسانتنا .و كمان علشان ربنا يكرمنا لأن ما عند الله لا ينال الا بطاعته
There are three levels that you have to go through them to be able to be evaluated in elective.

From the diagram, you can’t jump to step 3 without first and second steps .
1-language: you must be fluent in English as much as you can, at least to understand all medical conversations, and better to understand everything because you may interact with the team socially more than medically.
Methods: try to take conversation course at perfect place which will help you greatly, you can also to try to listen to English all your time, radio, songs, movies, and one of the most helpful thing try to get audio of any book you are interested in which helps you in listening and reading.
2- Health system: this is divided into: how is system + how you will interact .
How is system going: Ahmed el khanany included this in his note ( outpatient + inpatient + consult)
How you will work in the system: now you are working like students there, so we will discuss some points which will help to be at the level of them.
When you take a new case to follow up: this now is your case try to care about everything about it, for example if you noticed increase creatinine level in a patient : you have to search the internet for what are the causes of renal failure in this patient, treatment, prognosis . SO uptodate is your target. You have to search the internet also for any new methods of these aspects. If this done, this will be mentioned in LOR.
Try also to ask the attending intelligent questions, but take care !!! when you ask sophisticated question about hypersensitivity , you will get first an open ended question about what you know about all types of hypersensitivity and tell me examples , after you reply you will find the attending explaining for you for about 20 minutes for you only and this is very good situation, and this will be mentioned in your LOR. So try to read will in topic which you will discuss.
You must know that the attending is who will give you the letter, not the resident or the fellow. So try to be in contact with him as much as you can by: presenting cases or asking or attending conferences or journal clubs. But you still can get benefit from all the team clinically or in improving your presentation.
3- Your medical background: knowledge + practical :
Knowledge: try to read from Kaplan what you are going to take elective in, this is easy. It is good also that you study basic science of branch because you will go into many discussions about mechanisms and receptors, and if you replied this will be good and may be written in you LOR.
If you are in outpatient and obscure case came: so this is your opportunity: for example if you are at allergy clinic and epilepsy clinic came to the clinic for IV IgG infusion, so you should read about the syndrome and prognosis of this method of treatment. This is will prove that you have desire to learn and can deal with new cases. This will be mentioned in your LOR.
Remember you are still a student, he is not measuring your science, you not have to diagnose all cases at all, but you must show attending only that you are motivated and able to work at the system perfectly.
Practical skills: congratulation, in Egypt we learnt how to pick up fine clinical finding such as fine auscultation details. So you must invest this. The fellow or resident are very busy to search for murmur in all cases if not related to cardio, so if you find any finding Just tell the attending I found that, he will leave what he is doing and come for you and listen and this will be mentioned in your LOR.

How to get a good letter of recommendation and leave a good impression in a rotation
(Some people do a rotation and match from only one rotation in residency and fellowship)
1-Be the first to arrive
2-Be the last to leave
3-Work weekends if possible
4-Never be late it is better not to attend than be late
5-No cell phone ringer off no facebook in the hospital
6-Do any scut work when possible and be good at it
7-Do not leave when told it is late go home always stay to the end with your team
8-Dress like the residents
9-Good mouth and body odor but no perfume as it irritates asthma patients
10-Do not discuss politics or religion
11-Ask for the letter after at least two weeks of work with a single physician
12-Never look at the phone or answer calls in rounds
13-Add to the team look up information in up to date and tell them read on the patients and do not ask questions during work only ask in breaks or lunch time when no one is busy
14-Do not tell any one he is wrong alert them in a nice way or in private if there is a miss do not try to look smart residents may hate you
15-Smile all the time even if tired never complain
16-Speak good about your country and medical school never play the victim role it is pathetic
17-Always treat old patients like your parents or grand parents let them go first never step on their feet patients come first
18-Never complain always say good things about every one
19-Ask in person and politely for the letter
20-Allow enough time
21-Remind the letter writer
22-Thank every one before leaving and after leaving and keep the contacts
23-Get a nice souvenir like a small papyrus paper to be remembered
Do not try to impress just try to be a professional hard worker who wants to learn and help
